Defective power supply for BeagleBone Black

Hi,
I recently purchased a BeagleBone Black (BBB) and the power supply for it (5V 2A switching supply PID:276 SID:2A.1). It is defective (see below). Please respond with instructions for an RMA so that I can return it for a replacement. Thanks!!

On first power up, the BBB started to boot normally but hung when the desktop came up. This happened again. On the third try it came up normally and operated normally for about 5 minutes. It then went black and all the LEDs on the BBB went dark. I used a DVM and found that the power supply (without load) was putting out an unsteady 0.1 to 0.4 V level instead of the expected 5.0 V. I was successfully able to boot and operate the BBB with a beefy USB charger powering it through the micro USB input so I have confidence that the BBB is fine and that this is just infant mortality on the PS.
