Wave shield SD I/O error.

I have an Arduino Duemilanove with a Wave Shield from adafruit.

it was working well when i first ran daphc sample code few days ago,
and i ran the same code again and it gives me:

Wave test!
Free RAM: 688
Error: Card init. failed!
SD I/O error: 6, 0

or
SD I/O error: 1, 0
SD I/O error: 1, FF occasionally.


I've heard SD I/O problems often caused by poor soldering,
so i checked solder joints and fixed some suspicious parts, and its still not working.

I am sure my SD card and files in the card works perfect for the wave shield cause it was working before,
I have no idea why it's not working this time and i keep getting error messages.

The solder on some of your jumper connections looks like it might not have flowed well onto the solder pad. Also, the 4th pin on the SD connector looks suspicious. Often when solder balls up like that it is because it hasn't flowed onto the solder pad.
