Wave Shield Card Init. Issue

Hello Everyone,
I have recently purchased and soldered together the Adafruit Wave Shield. When I upload daphc.pde I get the following error:

Wave test!
Free RAM: 672
Error: Card init. failed!
SD I/O error: 1, 0



From what I can tell this means I have soldered something incorrectly. I have also tested both a 1.0GB Polaroid SD card and a 2GB ScanDisk at both 4MHz and 8MHz.
Attached are pictures of my shield if someone could advise me it would be much appreciated.

After going back and checking the shield design I see now that I am not able to use pin 13 as an output pin so I will have to unsolder that. However, in my testing I have not connected any of the pins including 13 to anything external so I don't think it should be throwing off my SD initializing.
Any input would be greatly appreciated.

Your IC4 is installed with the notch at the wrong end. It should line up with the notch in the silkscreen.

I have un-soldered and flipped the IC4 chip and have all my files on the SD card as mono wave files. When I run daphc.pde it not longer returns any errors however when I play back the files I am experiencing erratic volume changes as well as random static, popping and crackling when headphones are connected to the jack. I understand this is a rudimentary system which will not be capable of playing back high quality sound but I think these issues might be fixable. Is this the case or using could there be additional assembly issues that could be causing the audio issues I am describing.

There look to be some solder balls on both sides of the board. A cotton swab and some rubbing alcohol can help clean this up, or I keep a ratty old toothbrush around for the job. Check for any cold solder joints as long as you're going over it...I don't see any (except for the wire on pin 13, which you already mentioned removing), but sometimes photos don't capture these details.

Anyway...probably something in that quadrant of the board...the two 8-pin ICs, headphone jack, and the discrete components between all of these.
