Wav Shield issue with WAV Files - Help

I am working on making the Voice Changer but I am having an issue with reading the WAV files from the SD Card.

The SD card seems to be reading fine and if I run the example SDReadTest it seems to read the card just fine and here is the serial output.

When I run the adavoice code the voice changer works, but it doesn't play the startup.wav or any of the sample wav files. This is the output of the serial.

I have tried the other examples like PiSpeakHC and I am getting the same error "DVOLUME must be set to non-zero in WaveHC.h
Not a valid WAV"

Also note that the SD card was formated with the official SD card format utility. I also when through and have checked all the soldering and it all looks good.

Kind of stuck at this point. Can anyone help?
