Here's what I've looked into:

Voltage out of the regulator is 5.0v
Voltage at pin 7 of IC1 is 4.8v
Pins 8 and 22 of IC1 are grounded
Pins 15 and 16 of IC1 have no voltage

I've attached some pictures of the front/back of the board. I ended up taking the buzzer out to test. Added C10 and R2 because that's what the troubleshooting guide suggests. Any ideas?

The direct flash makes it hard to judge the quality of the solder joints. If you can shoot those again near a window in the daylight that would be helpful.

Soldering and component placement looks good. It is possible that your processor firmware was not installed properly. If you contact support@adafruit.com with a link to this thread we can send you a replacement processor.

Not to detract from Bill's suggestion but the soldering on the controllers socket looks a little iffy on a few pins. I also would re-solder the I.C. socket as well and see what happens.

I think I am experiencing the same issue here; the symptoms seem the same.
The voltage out of the regulator is 5.04v.
Voltage at pin 2 of IC1 is 3.98v, pin 7 of IC1 is 4.96, pins 15 and 16 of IC1 read 0.06v
I am not sure how to check that Pins 8 and 22 of IC1 are grounded, but I don't get any reading from them. (0v)

I did not find the troubleshooting guide saunter makes reference to, so I have not added C10 and R2, and I have left the speaker soldered on.

I re-soldered the pins on the socket and on R1 and the speaker.

Attached are pictures of the top and bottom views of the board. Is there something else that I can check?

@anro - The assembly looks pretty good so far. If you are not getting a beep, it is likely the processor. If you contact support@adafruit.com with a link to this thread, we can get a replacement out to you.
