Raspberry Pi 3B+ Boot Issues

Hi there, I recently purchased a Raspberry Pi 3 B+ from Adafruit.

Order Confirmation Number: 1861193-9311512619
Ordered on Friday, September 21st, 2018.

Upon arrival, the Raspberry Pi 3B+ booted up successfully once for a few
minutes, and then never booted up again.

I have read the boot sticky, I have tried a variation of tests, reflashed
multiple micro-SD cards using different tools, tried booting over USB, and
about four different adapters that meet the required voltages. I have used
multiple Raspberry Pi support forums, and even posted to r/RaspberryPi on
reddit.

I get a stable red LED telling me the power is adequate and the polyfuse is
not blown, but it just refuses to boot at all.

We would like to believe that your product passed your pre-shipment testing
and then proceeded to fail very shortly after.

I would like to return this Raspberry Pi 3B+ in exchange for another one. We
are a public high school Robotics team branching out towards Pi-controlled
robots. We look forward to order many more products from Adafruit for our
robotics needs, so we very much hope you can help us out with this.

At this point, I would like to return my Pi for a replacement.

What voltage do you see on the 5V and 3.3V GPIO pins?

I'm afraid I don't have access to a multi-meter at this time. I'll be able to get my hands on one on Friday.

Once I have the multimeter, what would be the procedure for measuring the voltage on those pins?

Set the meter to measure DC voltage of at least 5V, touch the black probe to one of the GPIO header's GND pins, and touch the red probe to the 3.3V pin, then to the 5V pin.

I got a reading of 1.5 on the 5v pin
and 3.98 on the 3v pin

Something is wrong there..

Could you post a couple of photos showing the two measurements please? 800x600 images usually work best.
