I recently purchased a Raspberry Pi Zero W from you. I have formatted 5 or 6 SD cards (ranging in size from 2GB to 32GB and from many different manufacturers, including a brand new Samsung Evo 32GB class 10 card) with the latest Raspian Lite (from July), but none of them boot properly. Sticking these same SD cards in my Raspberry Pi 3 boot up just fine with no problems. Some times the Pi zero boots far enough to load the kernel, but then gives a bunch of "mmc0: error -110 whilst initialising SD card" errors.
The Pi zero passes the "not dead" test, as plugging it into a Windows or Linux desktop PC causes a "Broadcom BCM2708 Boot" device to show up. So my guess is the SD card reader hardware on this pi zero is bad.
How can I request a replacement?

The RasPi’s microprocessor reads the SD card directly, so there may be a connection problem.
As a sanity check, what voltage do you see on the 3.3V and 5V GPIO pins when the RasPi is booting?

3.3v and 5v as expected. Using "rpiboot" or "usbbootgui", I am able to boot the pi zero into gpio expander mode over USB (no SD card is required for this mode) and toggle gpio pins from python on my ubuntu laptop (via pigpio). This proves the rest of the pi works fine, but really dislikes SD cards.

It sounds like there is something flaky with the connection to the SD card, and that's beyond any normal troubleshooting.
