There's a guide (https://learn.adafruit.com/qt-py-and-ne ... s?view=all) which connects the data directly to an analog pin.
But the NeoPixel guide (https://learn.adafruit.com/adafruit-neo ... -practices) states, that the data signal has to be 5V and the analog pin has only a budget of 3.3V according to my measurements. I wasn't able to get the external NeoPixel working.
Do I have to get a level shifter?

You may need a level shifter then. Using 3V for the data is not a guarantee and success can vary from product to product. There's more general information about using NeoPixels in this guide:
