Hello,
The support team I've contacted asked me to post my problem here.
My order number: 1538423-5497739065.
Order date: September 7, 2017 @ 5:51 PM.
Problem description:
I would like to get a replacement of my ordered Raspberry Pi 3 - Model B.
When I received it I tried to boot it up with NOOBS installer: it started,
showed the rainbow screen on HDMI output, then several lines of text and
that was it. The board itself showed red + flashing green lights, I left it
for 5-10 mins and then the screen went black. The green light was still
flashing, so I left it for an hour with no change. After that I tried to
reboot it, and since then it has only red light with no output or other
signals. I can assume, that it died after the screen went black for the
first time.
The micro sd card with NOOBS files on it got its primary partition resized
from 32gb to ~1,5gb, so it seems it started some installation process. I
also tried to reformat the sd card with placing exact same NOOBS
installation files, tried another sd card, as well as another 5v adapter.
All that with no luck. I still have the only red light and nothing else.
Please assist.
Regards,
Max

Re: Raspberry Pi 3 - Model B problem
That sounds more like a software issue than a hardware issue. Try downloading a copy of Raspbian and writing it to an SD card using something like Etcher:

Re: Raspberry Pi 3 - Model B problem
@adafruit_support_mike Hi, thanks for your quick reply.
I've just tried Raspbian + Etcher and got the same result -- the only red led.
Just in case I've attached screenshots of my SD card contents for NOOBS and Raspbian.
As I already said, the board was able to boot once for the first time (at least I've seen red+green leds and some HDMI output) with the exact same working SD card and exact same NOOBS files on it. I've also tried another SD card, formatting cards by windows built-in tools and the SD Memory Card Formatter from sdcard.org, 5v adapter from my ordered items and my phone charger, none of that helped. It seems it is a 100% hardware failure.

What OS image did you use?
There have been hardware updates to the RasPi line in the last year that broke back-compatibility with older OS images. The 'boots another RasPi' test isn't conclusive any more, even for boards that have the same name.

Re: Raspberry Pi 3 - Model B problem
I used the newest versions of both NOOBS and Raspbian images.
NOOBS Version: 2.4.4 Release date: 2017-09-08.
Raspbian Version: September 2017 Release date: 2017-09-07 Kernel version:4.9
I never mentioned 'boots another RasPi' test, I have only one board.

Re: Raspberry Pi 3 - Model B problem
I say unplug everything except power then boot and see if you get the flashy green light again. Might be a monitor issue of some sort. I have had problems in the past with a monitor that simply wouldn't sync with my Rpi 2 or Beaglebone Black properly.
Also try a different flavor of linux that starts with SSH enabled and no graphic output then see if you can log in through putty. If so then you know its related to the video hardware somehow (either the Rpi or the monitor).
I was actually quite confused after installing raspbian with a desktop and couldn't log in through SSH - thought something was broken! Turned out they default no ssh disabled with the desktop version.

Re: Raspberry Pi 3 - Model B problem
I've tried powering it on with power cable only -- same result.
Once again, as I said, It booted up first time. On the same monitor, with the same boot files, connected with the same cables.

Re: Raspberry Pi 3 - Model B problem
Sorry for the delay.
From the information you've given, there's a reasonable chance of an early hardware failure somewhere in the RasPi. Send a note containing a link to this thread and your order number to [email protected]. Those will be the RMA authorization.
