I've had a few RPIs in the past for various projects, this is my first time using a PI 4.
The picture clearly shows my issue. I have tried the following:
both HDMI ports on the RPI 4
two different HDMI cables that I've confirmed to be working on other devices
two different HDMI adapters that I've confirmed to be working on other devices
every combination of the two cables and adapters
a monitor that I've confirmed works with other devices
the signal cuts in and out but is off more times than on, getting this pictures was a pain and the PI is unusable.

That looks weird for sure. Have you tried any adjustments to the HDMI settings in raspi-config?

I tied the HDMI setting that shows during the PI setup. It's almost impossible to change settings with a screen as it spends most of the time as black, it'll flicker and turn off.

You can also change the settings by editing /boot/config.txt.
That file lives in the filesystem that mounts when you plug the SD card into a desktop computer. You can edit the file there, then move the card back to the RasPi.
