There is no USPS pickup on Monday Feb 15th, 2016 because of President's Day. USPS orders placed after 11am EST Fri February 12th will go out Tuesday February 16. UPS orders will ship as normal.

How do I go about contacting someone in Sales regarding an item that was purchased and worked only for a couple of weeks ?

I purchased a HDMI to VGA Video+ Audio Adaptor on March 23.
The Invoice No. is 256682.
I had it up and running for about 2 weeks and yesterday it stopped working.
No matter how I configured "configure.txt" I couldn't get a signal.
I then connected my RPi to my television in the living room, via a hdmi cable and it worked.
Has there been other reports of the hdmi to vga cable not working ?

We have had some reports of incompatibilities with specific monitors. But it sounds like yours was working fine until now. If you contact support@adafruit.com with a link to this thread we can arrange a replacement for you.
