Monster M4sk not recognizing USB - yes I tried all the things

Monster M4sk straight from the box will power up and run correctly. After following the directions to connect to a computer (Plug it in and hit reset twice) I get the following:
PC reports USB device not recognized
The red light on the mask flashes rapidly
Device manger reports: Unknown USB Device (Device Descriptor Request Failed)

Things I have tried:
Multiple different cords
New cord guaranteed to be data sync
Connecting through a USB 2.0 hub
Multiple Windows computers and a Mac (the Mac didn't see it at all)
Restarting the M4sk multiple times
Connecting the M4sk with power switch on and off
Checking for new windows drivers (It is trying to use the default USB driver v 10.0.19041.488 from 6/21/2006)

And now I come to you. Any help would be greatly appreciated. These are very beautiful props and I just need to do a little bit of customization to add a trigger response.

It sounds like you did go through all our usual tests, thank you. That saves time.

Let's check the physical hardware: post a photo of the component side in the area where the USB cable plugs in. We'll see if there are any missing or damaged parts. 800x600 images usually work best.

We received some additional M4sks and they connect with no problem. It looks like a defective board :-(
