Servo shield reverse polarity protection overheating

I have assembled two PWM/Servo shields (1411). One of them works just fine. On the other, the reverse polarity protection MOSFET gets really hot even without having an Arduino or servos attached - just a 5V supply. When I plugged in the supply, it took a few second and I started noticing the hot chip smell! The light for the power supply (green LED) is on and the polarity is correct. I checked the soldering and don't see anything concerning. Any ideas?

Can you provide clear closeups of both sides of the board around the FET? It could help figure out what the problem is.

what exactly was the power supply?

It was a 5V= 3A 15W supply.
